That is in fact basic bioengineering, yep. Biology is not limited to any stable structures humanity knows in the slightest, while addition of a third whole sex by necessity requires some new components.

Any point technically, but barring start from 0, here it's baseline female body pattern and baseline male body, on all levels, anatomy, genetics, hormones, and so on. There is nothing else that is ethically and aesthetically acceptable for creation act, so no genetic structures from any other life can be used by meaning or by function.
Then all off that stuff must be thought over to create a human with a female body but both sexes, that can naturally live from embryo to death and naturally differentiate, naturally breed with other sexes, and do so without errors or any emergent problems in any futa human, or any futa or normal offsprings she'll make.
Since existing bio-mechanics in humans are optimized for 2, not 3 sexes, it stands to reason that they must be either:
Reused via bio-interpretation by mechanisms themselves, any existing items must be utilized in entirely new ways to create and maintain results, via subtle and not so subtle alterations.
And/or there must be new mechanisms. At least a small amount of new cell types, new organelles, new signalling molecules (and naturally new receptors), new insert item of interest here.
New genetic code additions are a must, no base to fall back on.
That's only sex part. To make muscular futanari, comparable to men in robustness and without femininity decrease or loss of sexy fat distribution, fertility or what have you caused by bulk ups, just like in anime pics, one'd have to do even more of the same.
And naturally, brains.
Which means full scale bio genetic engineering, abysmal amount of analysis and post-analysis (environmental interactions).
