They didn't remain primitive. India had plenty of civilizations although for large parts of their history they were under foreign invaders' control.
Why they are so unhygienic today is about poverty and culture in post colonial India. Geography probably hasn't been the main factor in this.

India is, literally, a continent. It's like saying south america has natural barriers from north america, its meaningless at that scale.

The buff is only effective across distances which could be directly administered in ye olden days (1-3 days travel).

Internally, india has no region with sufficient natural barriers to help form the core of an imperial state.
