Realistically, how angular do human eyes get? For example, is angle on this picture realistic or is it just photo taken at an angle?

This is clearly a photo taken at an angle. You can tell by how the side of the skull is at an angle too.
Anatomically speaking eyes can angle quite a lot.
No idea what the limits are though. You can probably google that, just use the proper term, which is eye or palpebral slant.

Also, why is this information relevant? Just draw either what you see or what you think looks appealing.

Human skull is round. The more extreme angle in perspective, the more angular top eyelid will get, because that's how perspective works. Theoretically if angle is 90 degrees perpendicular to the camera, a slanted eyelid would appear vertical.
