not much honestly, certain cities were hit really hard during famines to the point where black markets rose up, mostly selling smuggled essentials from china, i.e food, the kim jong il cracked down really hard to the point where even some protests popped up in those cities, nothing violent though, just marches to local government buildings, kim jong un has been more lenient towards those black markets and these days they even smell cheap 'luxury' goods from china.
