The exact costs depend on the company, but for mine it is about $100 per month and the most out of pocket you can spend in a year is $5k, after that insurance covers everything. I had an injury in my hand (not work related) last year that required reconstructive surgery. I went into a hospital urgent care on a Monday, doctor checked it out, ordered an MRI for the next day, I got the MRI and off that he recommended surgery asap. A couple days later I get the surgery where they take parts of my arm and use it in my hand. After that just rest and physical therapy for 10 weeks and it was mostly back to original function. Even with all that I never even reached the $5k mark
