You used those things on Soyuz 2.1v with no failures so...meh. Most of the NK-33s flown worked despite their age so it's possible there was a random quality defect in that one because >Soviet manufacturing standards. Maybe the fact that several modifications were done for the ones flown on Antares did something while I assume yours were flown with the unmodified original hardware. We did a bunch of mods including new avionics and making them compatible with American-spec RP-1 fuel.

apparently those engines were meant for an improved N-1 rocket but the program was cancelled and the Politburo ordered them destroyed but some engineers hid them in a warehouse and they remained undiscovered until the 90s

The NK-33s used an oxidizer-rich combustion scheme (a novel scheme which was not attempted on US rocket engines until the 21st century) which necessitating coating metal parts with enamel to protect them from oxidation. It's possible that the coating on the failed engine got chipped or damaged at some point and the metal underneath was exposed and corroded from it.

they flew 18 total NK-33 engines (5 on Antares 200 and 13 on Soyuz 2.1v) with only one failure so considering the odds it could have been a lot worse. though i don't blame OSC for freaking out after the October '14 launch and deciding not to fly them again

I dunno, the final Titan II was almost 40 years old (since all of them were completed and shipped by 1965) when it flew. Didn't have any issues with that, then again it wasn't built with Soviet manufacturing and Q/C standards.
