The fertility question is a complicated one. Here in the US we have a belt dividing the western/eastern halves of the country that is weirdly high in fertility. They aren't poor at all, mostly rich actually (the blue counties you can see form a line). It seems like the defining factor is cultural, they're all religious and rural with big ranches and farms and so they pop kids out like crazy.
