If they are in a medical queue and they have to call a person by name, how would they do it?

Usually, people with the name Muhammad have a middle name too. Worse case scenario, their full name is used, i.e., {name} son of {father's name}. We also don't call out names in clinics or banks etc. We take a number at the machine on the way in and wait for the number to be called up.
