1. deterring invasion from a larger / more powerful country which may be interested in invading you
2. defending yourself against attempts at regime change / violent overthrow of your state by foreign actors
Then yes, a small nuclear arsenal makes sense. It's not really MAD, but it's enough to make both of those things dangerous enough that most governments won't take the gamble. For example, let's assume that Iran gets nukes, then the US invades. What's stopping them from nuking Tel Aviv? They've got nothing to lose at that point. Let's assume that the regime change / invasion is succesful. That would inevitably cause a window of chaos / anarchy in the country - what happens to the Iranian nukes? Who can make sure they don't end up in the black market, and from there in the hands of God knows who?
Look at North Korea, they've got nukes, and despite being sanctioned to hell and back, their regime is still kicking and has been for decades. I think you are missing the point of nuclear deterrence. Deterrence is not meant to prevent an attack, it's meant to make sure that in the event of an attack, the cost for the enemy would be so high that the attack would not be worth it. Also, the certainty of the retaliation is more important that it's effectiveness. Nuclear deterrence ceases to be when the enemy knows you won't be able to retaliate. This is why stealth bombers and nuclear submarines are so important. They are there to make sure that the country is able to retaliate even if it's been nuked first.

In the case of Iran, it's not as much a matter of having the nuke than having the capability to strike the US with it. Making a nuke in the 21st century is not that complicated, it's having a vector that is able to reliably strike any enemy even after an initial strike that is.

It's not a great power for sure but in terms of both population and area it's pretty big, don't let your parameters be based on a limited sample size of the largest countries. There's 196 countries and Iran is 17th in population, that's 91st percentile. An actual mid-sized country is around 34 million people, Iran has more than twice that.
