I think you are missing the point of nuclear deterrence. Deterrence is not meant to prevent an attack, it's meant to make sure that in the event of an attack, the cost for the enemy would be so high that the attack would not be worth it. Also, the certainty of the retaliation is more important that it's effectiveness. Nuclear deterrence ceases to be when the enemy knows you won't be able to retaliate. This is why stealth bombers and nuclear submarines are so important. They are there to make sure that the country is able to retaliate even if it's been nuked first.

In the case of Iran, it's not as much a matter of having the nuke than having the capability to strike the US with it. Making a nuke in the 21st century is not that complicated, it's having a vector that is able to reliably strike any enemy even after an initial strike that is.
