Besides it also limits the yield, the portable nuclear backpack the US designed in the cold war (W54) had a yield of up to 1000 tonnes of TNT, the one dropped on Nagasaki had a yield of 15 000 KILOTONNES of TNT.
A backpack bomb like the W54 would fuck up a strict city center for sure, but hardly do anything apart from that in reality.
