Combat medics are fare game, only the red cross is protected as they are unarmed and won't engage in combat.

War crimes are really only enforced by the implied threat of reciprocity. For instance, faking a surrender to ambush the enemy is considered a war crime because if you do that, the opposing side is now no longer able to trust that your forces are being genuine with any surrender offer. If you care about the lives of your soldiers at all, that's bad, because if some other guys on your side end up genuinely surrounded someplace they now have no recourse to stop the opponent from just shooting/droning them all dead. Better safe than sorry.
Russians have used Red Cross-marked fans to transport weapons. There's no international authority that can actually go in and arrest any Russian generals for this, so Ukraine has no reason not to blow up Russian casevac now. Hypothetically, the EU or America could tie aid to that but neither seems to care.
