I went the IC route 5 years ago and to the best of my recollection I had to fill out generic pre-screening mental health forms and was asked verbally to give a history of my GD, but that was basically it. After that I signed the papers and was told to cone back a week later for a follow up before being given my scrip. That's really not what informed consent is when it comes to HRT. You don't get asked to see a therapist. You don't get interrogated about your mental stability. They tell you everything that HRT will do, ask if you're cool with it, discuss fertility options if that's important to you.
