I think that the concept of gender identity which is defined by perception *does* take into account things like sports, medicine, etc. If those are your sticking points, then considerations of things like mtfs having an advantage in some sports factors into your perception of them, which influences whether you perceive them as men or women. The same with genitalia, and etc.
Sex is real, obviously. I *would* consider myself comprehensively educated in that, I did my masters in genetics. It is because sex is biologically real that sex can be changed. The things that make up sex are meaningfully real and that is why when those things are changed, sex then changes. Loads of things are real and also mutable.
