One is that most modern BDSM culture has its origins in gay leather culture, and of course this is the source of non-facial piercings, especially the sexual ones (which is ~all of non-facial piercings), which then spread to face piercings to some degree, most likely starting with tongue piercings which are also a sex thing. Similarly ear piercings in men came to be seen as gay originally due to 1960s drag culture (before that it was unironically sailor/piratey shit).
The other and actually completely unrelated reason is that normal mainstream people think non-ear facial piercings are ugly, and a large proportion (easily 90%) of """queer""" people are women who, rightly or not, feel desperately inadequate and unattractive, and who deliberately get the piercings to uglify themselves as a form of combined self-harm and revenge on the society that sees them as ugly, like "fine then, you want ugly? I'LL BE UGLY". This is why this type of piercing so reliably indicates the mentally ill.
