It won't ever work in the west because western countries are nanny states that are obsessed with preserving female fertility at all costs, they'll literally stop women from getting hysterectomy for severe endometriosis if they don't have a kid, doctors will literally peddle pregnancy as treatment, lol. And for this reason anything that's related to women's fertility is treated as dangerous, and women themselves are treated as cattle that can't be responsible for themselves, at least in this particular aspect of their lives. Hence all the "she's poor innocent girl who didn't know any better" despite talking about a 30something woman who decided to get a top surgery, then regretted it, and blamed the whole world, but herself for that. There's basically no accountability, it's always someone else's fault for not gatekeeping hard enough. You can't have OTC HRT in this environment.
It's not a problem in Thailand. Although it's also a nanny state, and abortions are illegal, and there's all kinds of bs. But when it comes to body altering treatments? Sure, go ahead, just don't try blaming others afterwards. Both T and E are OTC, 2 of my friend's son's classmates started HRT at 11 on their own. And yet there's no detrans panic. Your own parents will bitchslap so hard you'll get amnesia if you try saying you weren't gatekept hard enough. For surgeries, you get 2-3 interviews with different shrinks to get your letters and afterwards it's entirely your responsibility. But in the west, despite having similar requirements, you're still allowed to say shit like "uhm, actually, I lied and it's not my fault" and then suddenly everyone loses their minds.
Repression is still a thing here tho, it's a religious country after all, and socially conservative are plenty as well.
