I do understand that and I certainly don't think they're utopias or anything, but even in places like Pakistan and Iran there are still legal pathways to medically transition. I have a friend who lives in Turkey who doesn't have a lot of trouble with medical stuff and even got her legal documents changed. Those aren't arab countries but at least on paper they are muslim.
