I don't know about any specific case because I have never investigated the topic, but I know that along history, many cultures have developed the concept of "person who changes gender", such as indian Hijras, the thai Kathoeys, in medieval japan, ancient Arabia, roman galli... In the Bankans there is something similar, although I do not know the name, where they perform a ritual to swear to live as the opposite gender. The indians in america also had a concept, but I don't know about it other than its existence.
There has never been a global, unified concept of "transgender" until the 20th century.
Trans people have always been a small population. One cannot explain gender dysphoria, you have to experience it yourself to understand it. I think life is a precious gift, so changing one's gender to be able to live in good condition is a small price to pay for it.
