He laced his entire testimony in the stuff, he's so broken at the end that he was totally ok with setting America ablaze in genocidal furor through JP's system.

It's implied the Maelstrom that serves as the backstory to Harmony is this very incident, so go watch/read that if you want more fucked up Itoh bullshit, it's a ride and a half.

It's hard to tell because he essentially is the only person on the planet confirmed to share Hideo Kojima's batshit insane ability to string together high concept ideas into a mostly coherent albeit utterly batshit insane conspiracy thriller storyline, so much so that had cancer not done him in Kojima had Itoh pegged as a creative successor for Metal Gear.

That said, firsthand experience being kept alive by the miracles of modern medicine go a long ways to explaining his rather fucked up approach to weaving medication therapy into his storylines, I recall part of Clavis' losing it at the end of being potentially the result of the emotional suppressants he's on for his military career utterly fucking his ability to healthily process his mother's death, among a handful of other bullshits dealt to him through the events of the story.

Well, yeah, but to be fair Genocidal Organ is very thinly veiled Metal Gear fanfiction with the serials filed off.
It comes to a head with MGS4, which also reeks of Genocidal Organ and has Itoh handling the novelization.

It's not all bad, sure you lose conscious thought but you've probably had days where you flew by on the lizard brain just passively doing things without the up front thoughts firing off.
But yeah, Itoh was clearly giving a finger to the idea of simply medicating problems away, and that says a lot when he had long stays in the hospital dealing with cancer and having to rely on modern medicine to have a chance of surviving as long as he did. I sometimes wonder how much of his ideas came from staring down his own mortality, but we were robbed an incredibly haunted mind too soon with his passing.
