You can tell a mare is in estrus because she keeps randomly flagging her tail, and can be very craving of intimate contact. Unlike being simply clingy, a mare in estrus would also try to initiate coitus herself, wanting your penis inside of her, as her womb is active at that point, ready to be impregnated with your semen.
They have pills suppressing the estrus effects such as the extreme horniness, but if a mare didn't take them and you don't fuck her, then she would fuck you, and in that case she's going to put you in the pear-shaped hole for sure.
