First off, I think the number one thing that sex games have over traditional porn is interactivity and immersion. In order to make a decent sex game then, we need to leverage these advantages while I also balancing the fact that we need the game to be accessable for those that play with one hand only. So, lets start with things we shouldn't do first.
>Do not have a super long story, we don't have the patience for your VN bullshit romance. Romance is good, but keep it short and sweet.
>Do not add mechanics that force you to focus on pleasuring yourself, or playing the video game. Lets be honest, a very small portion of your audience actually want to play the video game, but rather want to have interactive sex. With that in mind, mechanics must compliment that.
Now with those general don'ts out of the way, lets talk about the dos.
>Allow player freedom first and foremost.
This will include which girls they will interact with, what fetishes they want or do not want, even those we may not like personally. It would also be a good idea to have the player's action carry weight and have a permenant or semi-permenant impact. For example, not using a condom gets a girl pregnant which means that she will have to to take care of a kid. This opens makes choices matter and might provide the player to interact with mechanics that may lead to romance, which may compliment the sexual mechanics. Or they can ignore it, but choices should be made avaliable and should matter.
>Voice acting is a must
Immersion is key, and characters not having voices breaks immersion, especially during sexual sequences. It also brings out personality in characters and provides variaty, which is an herb of living or something like that.
>Good art style + animation
This is probably the most important part. If your game looks like shit, it is shit, period. Sex is a very visual activity, and with porn, its basically the only sense you have. Also, no animation defeats the point, as if I didn't want animation, I'd go on some booru or something, I don't just want pics.
>Interactivity and control during sex
As stated before, interactivity is one of our big advantages, and thus we should use it in every scene. For example, certain sex positions or sexual actions, the switching of said positions, as well as speed, cumming, the use of protection, which hole, or even none sexual actions, such as staring into the lady's eyes, or hugging her. Basically, give them control during sex.
