A lot of metal's development involved becoming technical and narrative-written like prog, without the fluff that can get prog mocked. Incorporating the rest of prog into a genre which has long developed its own language and expecting the two to mesh well is, maybe with some exceptions, silly.
For one example, keyboards in prog rock can be cool, keyboards in metal (at least in prog metal) make me want to swirly the keyboardist.

Virgin steele had a lot of keyboards, but im not sure if they would count as prog (earlier on at least).

They had a few keyboard solos, but they were only on a few songs and mostly on earlier albums.

Not a regional thing, all of the most popular prog/psych black metal bands popped up in completely different countries.
Sigh was the first to do it and they had some "fame" worldwide in the black metal scene since they were on the same label as Burzum and Mayhem and other big black metal bands, so when they went prog they already had something of a following from black metalfags.

Other notable early bands were Meads of Asphodel in England and Oranssi Pazuzu in Finland.

I think prog black's psychedelic/-ish direction lends to the fact that black metal was never really a technical genre so it doesn't lend itself well to the wankier side of prog rock and pulls more from the noisier atmosphere of heavy psych, proto-prog, krautrock and some canterbury bands.
