How do you think people heard music before the internet existed? It had nothing to do with people caring, it had to do with it being the way music entered their ears. That's why radio and magazines and, later, MTV were so important.
And have you never heard of music scenes? The reason you know about Nirvana is that people in Seattle cared about music that wasn't on MTV or the radio enough to support numerous venues and dozens of local bands that created the sound that became grunge music. That's just a famous example but this shit happened all over the world in previous generations.
99% of old bands that are still listened to today exist because people cared about them before they got signed to a major label (if they ever did) or got played on mainstream radio or MTV (if they ever did).
