It's easier than you think for some people. I don't drive more than 3 hours on the highway without stimulants because I just start fading out and it's not safe.
3 hours in the city shifting, stopping at lights, and taking corners keeps me awake just fine but on a highway you can only really do top speed runs to kickstart your heart and that's a real gamble when you're half asleep.
