Noblisse oblige, at least being nobles always guaranteed them a leading position. They didn't have to be common footsoldiers or lead charges personally unless it was out of their own volition. Minor nobles from the 1700-1800s or close are way more deserving of respect, those people had every noble responsability but absolutely no benefit, yet they still fought whenever called for. The way you'd get a nobility title as a peasant was by distinguishing yourself in battle.

It was pretty bad in the 19th and early 20th century, the industrial revolution changed a lot of things. A lot of poverty, which is often forgotten by people who complain about "our" colonial history. Only after the war did things improve dramatically for the average person.

Medieval times are not really comparable to this, different challenges for different people, some things were worse, some things were better.
