a plane just lifted off from LAX and a faulty engine fire light came on, the pilot accidently shut off both engines and the plane almost crashed into the ocean. The pilot was able to perform an emergency restart and circle back and land. So, cutting off both engines accidently is easy, easier if intetnional.

If you watch the video from the cctv camera you'll see a lot of smoke of dust coming from the left engine exhaust when he rotates. They probably ingested FOD and got a fire warning then shut down both engines instead of just the left.
