Thinking back, I literally can only think of 1 instance for certain where it wasn't pilot error, or improper repairs or maintenance, or terrorist attacks. By the way, if he crashed it intentionally it was a terrorist attack.
I've seen planes drilled into the ground over a burned out light bulb. I've seen copilots lock the pilot out of the cockpit to ram the plane into a mountain. I've seen a crash with no survivors because one of those wasps that make nests out of mud made a nest in a sensor. And one time out of the like hundred episodes I've seen a plane just fell out of the sky once too, but it also exploded.

Because it's the only time in the flight where an action is unrecoverable, and now all airlines have a 2-person rule, there is no opportunity for a pilot to be alone in the cockpit and lock others out.

If someone cut the engine or tried to dive at any other point the other pilot would fight for their life and get another crew member into the cockpit to help.

He did it at the precise point where a few seconds of confusion would be enough to make it unrecoverable, and he tried his best to be subtle so the recording wouldn't make it too obvious.

Thinking back on major air disasters ... when there's been a mechanical failure, they generally are able to land the plane. Thinking of Alaska (?Hawaii?) air flight where the roof ripped off the plane. Even the one outside Kansas City (?) where they lost hydraulics and some control surfaces, they were able to "land" and a lot of people didn't die. Heck, they even land safely and without injury when the landing gear locks up.
But then there's the L-1011 that crashed on take-off outside O'Hare because a cargo door came off.

And to be honest I was convinced this was electrical before the report came out - Boeing had insane software buffer overflow issues that could cause loss of all AC and Air India had such shitty maintenance that it seemed like it was almost certainly a case of worst corp meets worst country.

We will probably never know 100% which pilot did it, probably the captain. We will never know who switched the fuel back on - maybe the suicidal pilot had studied/practiced in sim and knew the exact timings and just "slowly" restored fuel, maybe there was a brief struggle, maybe purposefully confusing voice remarks - so many possibilities.

The fact is, airlines have 1 job: to hire competent and stable people

The way to prevent this is to make aircraft as accessible as automobiles. Why aren't we all flying to the shops? Flying to where we want to go? In our own aircraft? Why doesn't McDonalds have a landing strip so you can pick up a burger? Big Aircraft have a monopoly but the power needs to be in the hands of the people like with automobiles.
