The official story is that he was a fart knocker and dumb ass that night. But personally I think there's something to the stories that Caroline had a really bad coke/Adderall/benzo issue, she freaked out on the plane over some women's reason, and caused it to crash. There was word at the time that Caroline wasn't dealing well with getting the fantasy hundreds of thousands of American women had at the time: landing JFK Jr. They say she was crashing pretty bad about it and didn't know why she was so unhappy after getting the fairytale. She was throwing so many meds/drugs at it but nothing worked. So they say she took everyone down with her that night.
