Unless you go to the shitty parts, going out of the tourist zones are generally better because in the tourist zones toy have beggers and grifters.
But I can't imagine how naive you'd have to be to travel to Africa and be shocked that there's extreme poverty there...

i learned that all places with socialized healthcare have private pay doctor offices surrounding the free hospital for some reason. how do they stay in business when its free next door?

They are either being paid by the free healthcare corporation which subsidizes its services partially or fully to its clients, or the clients are compelled to pay the full price to the private sector once they realize how shitty is the mandatory "free" healthcare.
It's kind of like when you're doing an annual vehicle inspection test for your car, and the place that does it is located in a complex with lots of other autoshops whether for flat tires, electric issues, etc.
They're all in it together.

It has a BIG impact primarily if you actually get to experience real local people who are not going out of their way to conceal or minimize the negative impressions of dodgy infrastructure, and people like corrupt police, shit food vendors with zero knowledge or care for hygiene, taxi drivers charging you 10 x the fare because you are obviously not local, sales people trying to fleece naive, gullible people who come from societies where there are systems like consumer warranty laws and such. Going on a cruise or having airport to resort shuttle service like pic related...
Wouldn't it be great if Somalis, Pakis, or Indians visiting functional nations, took some notes about tried and tested beneficial methods and systems, brought it back home, and dedicated their remaining days to implementing what they learned... The Nips did, South Koreans, and chinks in Singapore or Taiwan seems to kinda have figured out things, and possibly even the mainland chinks are well on their way... based on some of their most glitzy propaganda videos touring Chengdu for example... a city developers wet dream for sure, but built with monopoly money, likely with untested or subpar (or fraudulent, to save time / money) materials, designs and solutions, and will likely not be maintained like all construction needs, customarily neglected in China, as documented frequently on 4chan.
