When they offer you fillings vs removal. Vast majority to times the customer will decide (again and again) to get the cavities filled. And these need to be refilled too so you're effectively a nice cash cow for the dentist and it might be years before you decide "I'm tired of this, just remove it". Removing it removes the opportunity to make money for the dentist though.
The other thing with fillings is that they're actually damaging your teeth more (via drilling) to insert the compound, so guess what? Now you've made the problem worse and it will absolutely need to be refilled.
"yeah we're concerned your tooth might rot further and die, so let's kill it" - not remove it but effectively ensure that it will die and now you have a dead tooth in your mouth, which guess what? Is only going to cause you problems in the future. You will be back and chances are you'll want it removed.
The vast majority of cases where it's actually a concern, where you're in pain or the tooth definitely looks like it's started the process of rotting and it's only getting worse (despite best efforts). Then just get it removed. The only time you need to be worried is if you're starting to loose a bunch, especially on one side, that may cause issues with your jaw and you might need false teeth just to avoid that. The only time I'd ever consider getting like a replacement fake tooth or something is if it's at the front and clearly visible (like your top front teeth).
Things your dentist won't tell you: your issues with your teeth are your shit diet. Grains. Refined sugar. All these things mean you have to really protect your teeth. If you are on something like an all meat or keto diet your teeth won't rot. So what that means is those rare times when you do break your diet and eat/drink the slop, then you ensure you brush, floss. If you're disciplined in a diet that doesn't fuck with your teeth your teeth will stay with you until you die.
