If you need more doctors, the solution is not to go abroad to find alternatives that were 'licensed' outside capped US quota, but to raise (or just eliminate) the US caps.
Further, if you are failing to find graduates willing to work rurally (I doubt it) after relieving the caps you can tie an obligation to serve for a certain period of years in exchange for providing the financial assistance many qualified (but currently rejected) applicants would need to fund that education.
Sound far fetched? Not only was it a historically established common practice, it was the inspirational premise of the show 'Northern Exposure' where a poor kid from the bronx had to spend 4 years as a doctor in rural Alaska because of such a program.
If we were allowed to discuss the actual factors involved in these problems this would be trivial to solve, but it's all just one manufactured crisis after another whose intent is to flood countries with incompatible cultures so we become too distracted by that immediate existential crisis to attend the lingering frog boiling crisis' of the decision makers who necessarily engage in corruption and theft to fund their existences because they are incapable of providing utility in countries whose economies otherwise operate under a system of voluntary mutual exchange.
