If someone’s truly insane, mentally retarded or a state of psychosis, the text is saying they’re EXEMPT from having to make the necessary restitutions if they harm someone else. But not the other way around, the insane retard (who I guess they call the “shoteh” here) still has a right to restitution if a mentally healthy person harms them, the insane retard is meant to be protected, and the non-mentally-ill person DOES have to make restitution if they harm the mentally incapable one.
