I don't have a horse in this race, but from my understanding, those regions are full of ethnic Russians who speak Russian who got left in Ukraine by the new borders after the USSR collapsed. It seems to me that they will eternally be a source of trouble until they get to be part of the country they associate with. This is why you don't let people with loyalty to another nation become a large population in your own.

The US will likely cede much of the southwest to Mexico in the distant future due to similar separatist movements that overrun the native population. You saw a flicker of it during the ICE riots where spics were out there burning cars and waving American flags. Having a large population in your country that is loyal to somewhere else is ALWAYS going to lead to this problem. You either let them secede your territory to the country they're loyal to, or you line them all up and shoot them in the back of the head before repopulating the region with your own people, but that won't be viewed as an acceptable move in the modern world.

Those regions in Ukraine and regions near Ukraine in Russia as well like Kursk as well as regions going around Azov and up to don are not the same "central Russians" as Moscow is for example, well to be correct they may be by now because USSR and Russian federation had an effect of slowly creating generic identity similar to how people from different US states are more similar now than not, but in the past going as late as 50s at least according to soviet perepis naselenia huge portions of all of these regions spoke Surjik which is a transitory weird version of Russian that is between Russian and Ukrainian on a gradient.

The demand from spring was 5 regions and it's gone up since then. Losing just 2 regions was the demand pre invasion (well actually letting them vote to leave which would have lead them to leaving so basically the same thing). Obviously the later you surrender the more you lose in the peace deal since you had to waste resources to make them surrender you need compensation for that.
