It's probably liver spots, or a bruise he got just doing nothing. Humans are actually pretty careless and we ding ourselves all the time without noticing, because it doesn't leave a bruise. As we get older our skin turns to paper, so the bruises show more easily. As president he has to keep up appearances that he's in perfect health, otherwhise people gossip
