So I've checked this out, obviously Riyadh has a large population of people who fast regularly.
Giving him Glucose is actually the play if you're in a coma from fasting because your glucose reserves after 20 days (28 days in Miles's case) are literally gone and you need glucose for brain function.

I almost died to refeeding syndrome once. I was /fit/ through highschool, went to the military in early 20s, all /fit/, then got out in mid-20s and immediately just got fat. Fat as fuck. Went from 6'2 and 180 to 240 to 300 to 350 to finally plateauing at 400 and just staying there for years and through most of my 30s.

I didn't have the will to keep my caloric intake down, so no matter how much I worked out I couldn't hit a deficit. Being fat is hard because you have to keep eating to survive. It's like being a heroin addict and trying to get by on a little hit a day.

And so my doctor put me on an extended fast, which I didn't even know existed. I thought you'd just die if you went a few weeks without eating. And so, I drank mixtures of sodium, potassium, magnesium and water every day and just stopped eating. The weight vanished fast. My fast lasted for 9 months and I was down to 220.

Granted, there was more to it than that. You need to start taking protein supplements so your body doesn't cannibalism all your muscle before the fat, and I was on a ton of multivitamins to make up what I couldn't break down, but it really worked.

And then I got off the fast and almost immediately died because I didn't know what refeeding syndrome was. Your body is built for extended fasts. It's a holdover from an era when humanity didn't always have the ability to eat regularly. So, it can turn off burning calories and start feeding on itself, suspending bodily functions that you don't need. And so, when you reintroduce carbs and shit with no ability to break them down, food becomes literal poison for you.
