Depending on what you mean by fail. A once in a hundred years earthquake hits it directly? Could result in hundreds of thousands, maybe millions of deaths. Getting bombed wouldn't really do anything, the dam is over 100 meters thick made with reinforced concrete, you would need a nuke to seriously damage it.

this would not be an air burst nuke.
that said, an actual strike is quite unlikely, since there would be no plausible deniability.
more likely there are other ways to damage the dam in a way that would all but force the dam to be drained.
something like burning out the control circuits for the generators or somehow crippling the floodgates.
wouldn't surprise me if the cia had half a dozen different plans that would barely draw suspicion.

If it cracked like in the Lord of the rings, nothing would stop the floods, and millions would die. Curvature dams are much thinner because the curvature pushes water pressure to the mountains on the side. Gravity dams are usually straight, meaning that they will need to withstand the whole water pressure. To put it simply, think of the dams you saw in your life, and multiply the thickness by 10, and you have a gravity dam

Reminder all dams are temporary regardless of the integrity.

All dams create a still water deadzone because they are physically blocking the water. This sudden stop causes silt and other debris to be released from the suspension and become deposited at the floor. Eventually this displaces the reservoir's capacity itself, essentially dumping a mountain of sediment infront of the exit eventually blocking it.
