So governments are lobbied to promote reproduction to promote more fertility. Programs offer lucrative incentives to sign up. You fill out a lengthy form explaining yourself, your family history, your preferences, your interests, etc. then you are matched up with someone of the opposite sex with similar characteristics.

I don't mind the forms but that is rather inefficient. Western governments with aging populations like the US needs tax dollars now, not in 25 or 50 years from a theoretical baby boom if every single person is married and has 2.1 kids.
That is why governments are incentivized to have immigrants and introduce taxes immediately that way without the 18+ years to be a "productive" member of society. In a theoretical situation where birth rates have bombed across the planet then your proposal is better.
I think a better form for the modern era would be a singles tax. Have a government dating system where you have to go to x dating events every month until you meet someone. Then have a few years at most to get married. Maybe a 5 year pause at most on taxes with backpay required if you can't find someone in those 5 years. And more benefits for marriage other than the current tax system. Home loan programs like 1930s Germany with 25% of loans forgiven per child with loan payments limited to a maximum percentage of your combined income. Encouragement of home gardens for the women to care for while staying home with children until school age. Execution of nonwhites also would be nice since I only agree with socialism when it benefits my race.

For each state mandated gf assuming its random it would only be a 25% chance of happiness and a 75% chance of making things worse.

It would create more. You need women to want undesirable men. Only real solution is to start addressing the financial side of divorce laws like Kentucky and China did and also make birth control pills prohibitively more expensive.
