For gimmicks involving someone coming out from under the ring, is the wrestler just chilling in there the whole show and they go under there before the doors open for the crowd?

Yeah, they got a little monitor down there and usually a production guy to keep him posted and chill with him. Mox talked about it in his book, when he was down there the whole show waiting to cash in his MitB. You go in disguised as one of the ring crew guys and slide under when the others are crowded around you and hope no one sees.

There have been instances of a wrestler staying under the ring from before the show starts but usually most of the time they'll sneak down to the ring in a hoodie or something during an intermission or while a promo is playing and the arena is dark

Theoretically you could bring a cart of t-shirts or whatever down to ringside and park it there during a commercial, hand some out then skedaddle before the live feed goes back on air. In that time a wrestler hiding under the cart could slip under the ring undetected. Idk if they ever did this in wrestling though

There's a little elevator under there. When it's about time for the wrestler's spot he takes the elevator to discretely get beneath the ring.
