What he’s saying does make sense in a retarded sort of way specifically because it’s an I Quit Match. Every time you are asked if you Quit you’re being removed from danger. Sticking your head under water for a few seconds doesn’t hurt enough to justify quitting and when you’re asked if you quit you’re breathing normally. Mix isn’t going to drown him because he needs him to say I Quit. You don’t win by murder, you win by the other guy verbally quitting which cannot do if you are drowned. It’s a stupid spot especially for this type of match

It’s “simulated” in a very specific way, the “boarding” part is actually just as important as the “water” part and the simulation part, pouring water continuously over someone’s face while they are covered with a rag, triggers involuntary reactions that are literally torture. Sticking someone’s head in water actually doesn’t do nearly as much damage to someone.
