Bruno walked out the door against Hansen in the 70s, other people having been walking out the door since the 60s. Going out the door has probably been the default finish a majority of the gimmicks lifespan, either by beating someone half to death or crawling/falling out. Shit more people won via pinfall or verbal submission than ever went over the top.
It only started getting more popular to go over the top once people realized people diving and taking bumps off it was more entertaining, even more so when interference en masse became a fucking staple for it. Even then, it has never been a thing where going over the top has been the only way to win (unless you're in that retarded ass domed Mexican cage).
