Has anyone had success creating image to image gen of bdsm ball gag? I've tried some on civitai, but the results are poor or look too much like anime. Looking for realistic outputs.

What Bag said exactly. The lora also tends to work fine with realistic pony models. It often draws balls with irregular holes, so either accept that or be prepared to cover it up with another inpaint at the end.

Mind if I point out a skill issue here? This looks like you just painted the gag over the mouth without adjusting the chin: her lower jaw didn't come down, suggesting that her mouth isn't open behind the gag.
