How does someone get so mentally ill that they start dressing up as a woman, pump their body full of dangerous hormones and have their genitals chopped off?

I mean, I am a depressed shut-in NEET virgin, but even in my darkest thoughts, I would never in a million years even consider trooning out, and I recognise that the people who do it are destroying themselves.
