Just going from memory, the short version is something like: a little girl (Momoka) stopped a terrorist attack by Penguinforce in 1995, changing fate in the process; she and the leader of the Penguin Force had their souls split in half, and one of her halves ended up in the hat; her younger sister (Ringo) found a diary describing her original fate in detail and tried to live out its contents exactly to "become" her; The power to change fate ended up in Shoma's hands as a kid and he used it to save Himari and Kanba from the child broiler and his family respectively, and make them his siblings; Himari was then fated to be part of the Triple H idol group with her friends but that fate was overturned by her terminal illness and her friends formed Double H without her; Her lifespan is temporarily extended by the hat but the only way to save her in the end ends up being splitting the world into 2, with Shoma and Kanba in one world and Ringo and Himari in the other.
Also Kanba's original sister is going around erasing his exes' memories, not sure what that was about.
