Thread question: what is your personal strategy for addressing patients who misunderstand the role of pharmaceuticals in health? I.e. "I just dont want to take so many pills" in a post-MI patient with a stent.

For common non-narcotic drugs and stuff with really short half-life, like adenosine, how much does IV push rate really matter? Almost every nurse I've seen when demonstrating a slow push, does it over 10-15 seconds while claiming 2 minutes have passed. I'm pretty sure I've seen a lot of cardiac issues rise up because of this. In the few anesthesiology procedures I've seen, the anesthesiologist more or less slammed all of their meds, but I don't really know how long they're supposed to push for their meds. The only time I've ever seen a true slow push was hypertonic saline by the neuro residents.
I tried to have a conversation with non-med people and explain the pain of midlevel creep, and they all essentially called me “elitist” and “duped by the system” and said “I really don’t care who signs off on my meds as long as I can get them”

Midlevels objectively have less training, experience and intelligence, 3. They're more likely to suffer malpractice or die from a midlevel and are guaranteed to suffer a worse outcome according to The Science(TM)
Just narrow it down into digestible logic they can stomach one at a time

I mean shit dude I went to a derm at a hair clinic lately because the Norwood reaper is starting his harvest on me and i decided to hop on fin, after a year of min. I was hoping for him to maybe illuminate me with some obscure knowledge, being a derm at a fucking hair clinic, then he unironically argued that min inhibits DHT production too. This is exactly the kind of guy that should be a midlevel. At that point i was like "alright whatever just give me the fucking meds retard". In Argentina yes, you gotta do residency before you're allowed as a nurse/doctor, but for most of the career you're mostly studying from seeing dead bodies, and you don't go every day of the week to college since a lot of people need to work to survive. A lot of people study from books and assignments and PDFs the professors give, and exams are eliminatory. Learning by memory is more fomented over really understanding what does what with this kind of fast, chaotic system. In some parts of the country the career is free, or paid by the state. The issue is that the system is quite flawed and you'll end up really learning better once you're working under someone.
I did like one year of med school, but couldn't get through due to mental health issues, and I can't sustain myself studying for 7 years. I'll try to get a career first as a surgical assistant, which takes less years and has a decent pay, and they're in demand, for the moment.

That's insane but I've heard similar things are happening even in places like the NHS in the UK which is why their students really struggle in residency here (US). I feel like I've only heard of South Asians (Indians, Pakistanis) having a similar level of clinical experience to American med students with actual ward-based rotations and observing surgeries. Here you're expected to devote your entire waking life to medicine (and cut down on sleep for that matter) for 4 years, and even more so once you enter residency. Intern year (first year of residency) is phenomenally brutal in every specialty, but especially surgery.

There's nothing to feel like, that's literally how it's designed to be. Med school isn't to teach you how2doctor, it's to teach you the bare fundamentals you need to even know what the fuck is going on. It's like learning traffic laws before you sit in a car to learn driving.

Oh damn, here you can't really do residency if you're not really advanced in your career or at least have your degree, you need a legal certificate that allows for you to treat patients, or else there's gonna be some REALLY BIG issues with the hospital. Private colleges are more intensive and have more days dedicated to classes, but here, at least in la Universidad Nacional de La Plata (UNLP) you sometimes had to go like... 3-4 times per month for a single assignment, and like an hour or two/three. And there's "optional" assignments you can assign yourself to, to add more experience in your career. They're "optional" but you gotta do a minimum number of them by the end of your career. So it's like 4-5 assignments in a year, and you study at your own pace. The issue is passing the goddamn tests. Niggas get raped in exams here. If you fail ONE single exam you have to retake the entire assignment next year.
>>16767284
In some places, like my country, yes. It's designed like that. But in other countries med students have to actually be in the hospital and treat patients together with doctors, like the US I've heard.
>>16767369
LET'S FUCKING GO LOCO, here in some colleges the minimum to pass is 4/10. Getting anything over a 4 is a glorious victory, fucking enjoy it man. The most important thing is to just understand what you're studying and relearning it so that you don't forget it in the future.
Also don't fucking abuse caffeine and tobacco like every med student does, try to end up alive by the end of your career.

You shouldn't have to finish residency to build a basic community practice with medical school knowledge.
>inb4 that would make you incompetent
That's exactly my point. Med school used to train you to be a competent doctor in all fields out the gate without a need for further training, but now so many students don't give a shit about learning stuff beyond the test because "hurr durr I'm going to be specializing in pediatric neurocardiac surgery so why would I need to know OBGYN thoroughly" despite being given the information to actually be a decently competent basic provider. That's why I actually appreciate the UK pathway - EVERYBODY does the same foundation training after medical school and can be a competent GP straight out. For the US, it's a fetishization of being hyperultraspecialized that keeps Type A status-driven students idiots in anything beyond their specialty - which is why you have orthobros that can't even remember their fucking antibiotics beyond ancef or how to manage diabetes despite being "the top of their class"

But we know which came first between the chicken and the egg, residencies were established for so long before insurance and current licensing regulations came into effect that it became the standard for licensing to have completed at least a year of residency to receive your license. Same goes for that insurance coverage. It used to be a plus to have a residency. Now it's a red flag to not have one

In the case of IM, its patients you spend days treating for exacerbations of chronic illness only for them to discharge, never follow up, and ignore your advice leading to a slow decline that becomes increasingly frustratingly readmit and die

There are some perks though. The shitty hours can work in your favor too, there's no follow-up so you can for example work straight for 2 weeks per month and neet for the rest of it. You also don't bring your work into your home, mostly.

here's the deal if you can't figure out still retard. You can ensure eugenics in an egalitarian fashion in the following way:
You can do that by Ensuring chads & stacies breed with each other like wild rabbits while simultaneously donating their gametes to men and women in the bottom rung of the genetic ladder so that there's a sort of upward mobility for those at the bottom genetically speaking. Couple this with embryo screening and you can accelerate human evolution like mad. Such a system literally ensures a participation of all people in society because they know for a fact that their hardwork will be rewarded with genetic survival & genetic mobility and as a result you just have a lot more productive populace who inturn will have children better than them of their blood and so on. Guess you're too mentally dull to even think of this and perhaps you should be the first one to get castrated & worked to death for the overall good of the human species as your strategy is both retarded darwinistically & just downright evil.

Daaamn, must be the asian genes but you're smart as hell to be able to do engineering and medicine, and even anesthesiology. We don't need MORE people when we can't even manage our resources without corruption and pollution fucking everything up. We need better living conditions first, and then having kids and dating becomes a priority.
And if you don't want to improve you won't get what you want, simple as. If you want the ideal woman you should offer something just as good as what you're looking for. And even then, idealizing people will just end for naught. Stop looking at the outside and look on the inside for a second.

Why would we subject countless generations to suffer for the sake of your genetic ladder? Just do society a favor, keep those shitty genes to yourself and die, freak. Be glad you get to die peacefully at least, back in the day genetic dead ends would have just died in a war or from war/starvation, assuming your mother wouldn't just leave you in the forest to be eaten by wolves the moment she saw your malformed body. I didn't know I would enjoy the medicine aspect of medicine, now I do. I didn't know I would really enjoy the pace and culture of the ER. Etcetera.
Every rotation I did, the parts I enjoyed were in EM and the parts I hated were not. I was between ortho and EM and realized that, if I was an orthopod, I would miss the medicine side and the ortho stuff wasn't enough.
I enjoyed OB/GYN but knew I wouldn't enjoy the hospital side enough to make the clinic time worth it.
I enjoyed outpatient Psych, but I wanted to do real medicine.
I enjoyed the trauma resuscitation in surgery but I dont want to be a surgeon.
Your experience will be different, pay attention to what you enjoy and see if you find a specialty that captures enough of the good to make the bad tolerable.

I hurt my balls masturbating I went to the ER and the doctors there gave my balls an ultrasound and diagnosed me with varicocele on my left testicle and prescribed me liquid doxycycline 20 mls because it hurt and my leg and lower abdomen was tender. I am currently on the liquid doxycycline but the infection hasn't gone away and my balls have been hurting this whole time, before they were completely hurting, now they only really hurt if you touch them. My left testicle is swollen. I followed up with my primary care provider after the ER visit and he didn't prescribe me any new antibiotic when I run out of doxycycline and he referred me to a urologist. But I am like a week into taking my antibiotic and the infection in my balls has not gone away. I don't have the referral to the urologist yet. What should I do?

1) You have no idea who is or isn't a doctor
2) even if we are a doctor, we are not your doctor and you have no follow up with us
3) no physical, no reliable way to take a true history, no imaging, no labs
4) if we did, people would come here instead of going to a doctor
If you're really worried, go back to the ER or schedule another appointment with your GP if you dont feel its an emergency.

I guess it's useful to discard false positives or negatives, or to correctly diagnose people if they come to develop a certain condition or illness. Mostly to differentiate on what to do, what NOT to do, who to derivate to, what to use... I imagine specialties like Infectious Disease might have to keep up on all the weirdest, niche'st knowledge because it's their specialty diagnosing very specific and hard to treat cases.
It's good to know a decent amount of the basics of everything in case an emergency comes, or like for example if something related to neuro affected the heart, or kidneys with the heart, stuff like that. I'm not a doctor though.

Giving advice is off limits but dude, go to the ER/doctor again, and stop masturbating for the time being. Varicose veins and inflammation of veins don't go away that quickly, maybe you might need surgery. It's common in varicocele for the left testicle to be more affected. It's probably swollen due to the nature of blood flow getting blocked in a blood-sensitive area.

I doubt you'll be unable to masturbate if you were to lose a testicle, I mean, but... It's not guaranteed you'll lose it over having varicocele. Let your dick rest dude.

>Why would we subject countless generations to suffer for the sake of your genetic ladder?
my offspring will literally be far better than me if I introduce stacey's genes in my bloodline though. I'm not demanding chads to handover stacey to us. If anything, I want chads & stacies to breed with each other to produce pure blooded chad-stacey caste folks. What I'm advocating for is a sort of trickle down eugenics where chads and stacies, in addition to creating pure blooded chads & stacies, must also donate their good genes to folks at the bottom so they can create half blooded chads & stacies in generation 1, 3 quarter chads & stacies in generation two and so on. Only insecure men and women who fear that the folks at the bottom may end up creating much more chaddier or stacier children, who could inturn pose a threat to the bloodline & status of folks at the top, will fear such a system.
>You participate in society because you like participating in society
everybody participates in society for physical and genetic survival faggotard. You help others and in exchange the society helps you to survive. It's kind of like the individual helps the overall collective and the overall collective helps in the individual's physical & genetic survival prospects in exchange. It is an unwritten law that is now being broken.

You are at no real disadvantage. In fact, many schools these days are looking for students like you who have a career change, life experience, etcetera. Look into what it takes to get into medical school, you will need to go back for some prereq classes but by no means do you need another degree. Do some shadowing, investigate the profession, learn if it's right for you and then do well on the MCAT and send your application. I am sure you could write a personal statement in a way that tells an impact impactful, interesting story.
Okay now go away pre-med

If you can't comprehend even after such elaboration, I think subhumans like you should resort to seppuku asap and cease wasting any more usage of oxygen than necessary. Lol. Guranteeing genetic survival for all, in a eugenic fashion, literally means you would end up with a lot more brainpower to achieve what you want as a civilization. You also end up with a diverse, and still a eugenic genepool. You also provide every man and woman a stake in society's future because children are the future and will do whatever it takes for the future to be bright. Moreover, if some random disease were to infect humans, the odds that your genepool will endure against that disease is now a lot more higher , compared to a genepool only generated by chads banging all the foids and leaving behind billions of men.

Lobotomies were of course barbaric. JFK's sister had a lobotomy done to her, and it destroyed her life.
Today's psychiatric methods (antipsychotic drugs, electroconvulsive therapy) aren't as bad as lobotomies, but they're still shit. They have significantly harmful effects upon the human body.
>>16776210
>Because laws are made for the good of the society
Protecting society obviously makes sense, but we don't need to completely abuse psychiatric patients in order to protect society. For example, potentially dangerous people could be locked up without drugging them. Forcible "treatment" is probably the worst aspect of involuntary commitment, rather than the commitment itself.

Hey /med/. I was wondering if you guys could help me out with something. My grandma has some blood condition but the doctors are unable to find out what it is even after multiple bone marrow biopsy. She has progressively lower blood count (red blood cells, white blood cells and platelets). They did some CT scans and found nothing and did a genetic test that showed "a minor
change that can be seen in healthy individuals as well as in various blood cancers, and
therefore does not give us a clear answer at this time."
What could it be? Some of my family's doctor friends said it could be a viral infection but they haven't seen any of her results.

Average is only half of the picture.
Sun exposure, exercise, and early social dominance are all essential. If you're a macho preteen, successfully bully your inferiors, and slam puss early you get a big dick.
Your status (and therefore hormonal feedback) during puberty determines your degree of phenotypic masculinization.
In other words, small dick is a skill issue.

Mfw osteopathy was invented as a new way of raping women
>https://www.latimes.com/opinion/op-ed/la-oe-heffernan-larry-nassar-20180126-story.html
>These practices include intravaginal manipulation. Fisting. This was the “medical procedure”
>According to his victims, Nassar’s attention wasn’t on their hamstrings or ACLs; instead, he focused on their anuses, breasts and vaginas. In January 2017, one victim spelled it out in her complaint: “Nassar digitally penetrated Plaintiff Jane A. Doe’s vagina multiple times without prior notice and without gloves or lubricant.” Other victims describe Nassar’s forcing his “dry fingers” into their anuses and vaginas. The violent fisting was excruciating.
>Then came the gaslighting. When the girls blew the whistle, Nassar and his enablers tirelessly reasserted his privileges as a doctor. “We were manipulated into believing that Mr. Nassar was healing us as any normal doctor is supposed to do,” Capua testified. Just last year, the American Osteopathic Assn. released a statement to MLive.com, the Michigan news service, saying that intravaginal manipulations are indeed an approved, if rare, osteopathic treatment for pelvic pain.
>No need to fact-check that. Grabbing a young woman inside the vagina is not a first-line or thousandth-line treatment for anything. The victims knew this intuitively, but over and over they were told to doubt their perceptions, suppress common sense in favor of mystifying quackery and accept their unerring reflex to recoil from Nassar’s probes as their own failure. The takeaway was this, to paraphrase the president: When you’re a star doctor, you can do anything.

Face it guys, psychiatry is a crock of shit
With most medical conditions we know that something is physically going wrong with the body, right? Type 1 diabetes is where the immune system destroys beta cells which normally produce insulin, right? (Yes I just read Wikipedia)
Psychiatry isn't like that. Psychiatry claims a patient has a physical brain defect, but it doesn't PROVE they have a brain defect. Where's the medical imaging showing that a depressed person has a brain defect? Maybe their brain is pretty normal, and instead they're unhappy because of certain life events? Maybe if you went through those events, you'd be unhappy too? Maybe they don't want to tell you about all those depressing events because they might be embarrassed or ashamed about those events?
Conclusion: psychiatry is nonsense

Why would you do this besides as a cope for bad GPA/MCAT or being too old/poor? The jobs really aren't that similar besides working together in healthcare settings. I can't really imagine someone who was already in med school deciding to drop out and switch to doing med passes and wound care all day. If you're already in med school, the shortcut path to NP/CRNA makes no sense since you'll have to start again from scratch and take on more debt and lose more time taking nonsense BSN/MSN courses.

Like I said, it makes no sense in most situations where the bar for admission to med school is high. If you just wanted to do the nurse skills in some outpatient/prehospital setting, I'm pretty sure every basic nursing skill is already within the physician scope of practice. Also, a lot of healthcare settings would be reticent to hire a nurse who might try to practice medicine out of scope. The only situation I can think of for a current student/MD is if they know they're going to wash out of med school or at risk of losing their license. Thinking about it some more, I guess I remember reading about Caribbean grads becoming NPs after failing to match to a residency but I think that falls under cope.

basically
>shit insurance
>want to get tested for ADHD/get some form of therapy (different issues)
>get referred by my insurance of in-network places that can test me
>ok
>look through list
>first 5 are all booked for months, both for testing and psychiatric help
>first guy that isnt booked for months and actually responds does tests online
>i dont know any better so I take the appointment, need to pay almost 1k out of pocket because of insurance
>go to psychiatrist after online psychologist diagnosis me with some stuff
>go to psychiatrist with this info
>he seems extremely annoyed and has a glazed over look when I tell him this
>tells me these tests are not valid whatsoever and interrupts me repeatedly when I point out the guy's credentials, the insurance referral etc
Did I do something wrong outside of being ignorant? I really didn't mean to waste his time but I left feeling like I did. I figured I went through things the proper way, idk

Fuck psychiatrists.
I reduced the dose of my antidepressant because I want to ultimately come off it. But a few days after reducing the dose, I'm now more lethargic and I'm finding it more difficult to do things and concentrate.
Psychiatrists push you onto these shitty drugs (and they can drug you against your will if they put you in mental hospital). Then your brain becomes DEPENDENT on these drugs, so if you try to reduce the dose, like I'm doing, then you get withdrawal effects, like I'm having.

Psychiatry is abuse.
If someone has cancer do you force chemo on them? No. It's their choice. Every patient should have the right to refuse medical interventions if they wish, whether they're a psych patient or any other kind of patient.

I bet it's a very small proportion of psych patients who are actually dangerous. Quite a few psych patients get locked up because they're apparently a danger to themselves rather than anybody else - e.g. they're considering suicide. Maybe we should be able to help such people without forcibly detaining and drugging them. I think suicide prevention phone lines are good. Someone who is considering suicide can phone someone and have a human conversation. Instead of the suicidal person being forcibly detained and drugged.
I'm hopping from general to general to ask the same question from software engineers, academics, and more and I wanna know how /med/ feels:
Physicians, would you want your kids to enter medicine, or do you think the field isn't worth it anymore, or that it won't be worth it by the time they're picking a career?

Yeah, even closer to killing every single one of their patients

Every study on APP outcomes has been done when APPs practice under physician oversight and on uncomplicated patients, they are so full of confounding variables it's a miracle they're not considered less valuable than a poster presentation.
