Taking Medieval 2 as a base:
>map at least this big (preferably a world map), with more depth in each area than Medieval 2. Some strategically important smaller towns as well as larger cities etc.
>all the appropriate factions and units to fill it obviously.
>mod support with uncapped regions/factions/units/map editing/etc.
>bring back Jeff van Dyck to do the music
>adjust the songs to be appropriate to the folk styles of the region you're in, e.g. middle east, mongolia, etc.
>can sail up rivers
>expand the marriage system to be actually useful
>greatly expanded merchant/trade resources mechanics
>ability to prepare things like ditches or wooden fortifications if you've been in a location for more than a turn with enough units.
>UI in the art style of the faction you play as.
Also, incorporate the good features of the later games
>historically accurate city layouts
>changing religion mechanics
>improved diplomacy
>resource system
>minor factions instead of rebels
>actual naval combat and amphibious battles
>siege units on walls
