That's definitely what it's meant to be. A visual metaphor for his guilt literally eating him up inside. I also like the idea that if it has to relate to the night Mari died, than this thing's design was taken from when she landed right in front of him. Eyes open, hair splattered around like tendrils, mouth open from the scream she let out. Significantly traumatizing for a 12 year old to experience.
