I have visual-olfactory synesthesia and she smells like bologna and coconut body wash. Pretty disgusting combination, can barely look at her.

No one knows for sure. If we play into your framework, where people reincarnate on earth, then it would be safe to assume you'd be reincarnated on earth as a default if you're unconscious. This would mean that in order to have a chance at changing that, you'd need to remain conscious so you could make choices that would change that.

In my opinion, in all these NDE experiences, they haven't died yet therefore they're pretty much relying on time dilation to experience what they're experiencing consciously while their brain is still running. However, if one were to actually die (biological death, no brain activity), then we could assume retaining a grasp on your ego and conscious state could become much more complicated, if possible at all. But for the sake of your framework let's assume it is possible to retain the ego and a conscious state after biological death. I don't think most people would achieve this, and they'd be reincarnated on earth as default.

To retain the ego, coherence would be mandatory, as well as an anchor, a strong sense of identity. Most people don't know themselves well enough and live their lives in a constant vortex of noise, this translates into lack of coherence and a weak sense of identity. So my guess is most people's egos would dissolve during the transition of true death. Meditative and cultivation frameworks would help with coherence and identity, so in this scenario it's safe to assume people who have practiced any of these frameworks would have a higher chance at making it through death in one piece.

Let's say you "cross over" in one piece, retaining your ego and being conscious. I would avoid trusting anyone or anything you encounter, you'd be ignorant. I would also design a geas beforehand, there's a good chance that in a "space" like the "afterlife" things work more "conceptually", so geas are probably usable. So if you come across anyone or anything, you can offer them the geas so they can't lie to you.
