Granted. Every time you wear a set of clothes (and it has to be the full set), your appearance will change depending on said clothes. Depending on your preferences, similar sets of clothes might lead to an identical transformation or to different forms; for example, wearing different types of pasties will probably give you the same curvy bimbo form.
Your form will always be female, no matter the transformation. Wearing anything resembling a Japanese school's physical education outfit (white shirt, blue shorts) will revert you back to your default tomboy form. Cosplaying as a female character will transform you into that character, while cosplaying as a male character will transform you into a genderbent version of that character... Think Naruto or Ranma 1/2. Your mannerisms will change depending on your form, however your actual personality does not: you will always remain in control of your form.
And yes, just like you asked, if you undress and get naked, that will not transform you into a different form: the transformation only happens when you put clothes ON yourself.
Unfortunately, you suffer from a pretty weird form of prosopagnosia: to put it simply, you cannot differentiate between different clothes. For example, if you look at any kind of headwear, all you see is a baseball cap - always the exact same model. Same goes for clothes for your upper body (t-shirt), clothes for your lower body (shorts), underwear (pants), shoes (running shoes) and so on and so forth. If someone else is wearing those clothes, you'll be able to recognize them just fine; but if nobody is wearing them, or if you're wearing them yourself, you're shit outta luck. As such, unless you are VERY organized with your clothes, you'll probably end up wearing random stuff, with hilarious and often contradictory results.
