the reason the traincar is so bloody where Gem is, is because that's where Geppito took and killed Lea for her Petrified Ergo to put into P. Thats why Gem is there, he was awakened from his coma from that Ergo. Gem was still in his coma in the DLC, he is on Leas back (for some reason she never mentioned it)
the last bit of the DLC is pretty close to the beginning of the base game, I thought it was years ago but it was only like a few weeks at most. Romeo is taken to become the King of Puppets, starting the Frenzy, and soon after that we awaken
