They use earpro with built in noise dampening, basically the same tech you get in a pair of Walker Razor earmuffs. They have a noise-cancelling feature so low-decibel sounds can be heard (i.e. talking to a guy next to you), but anything over a certain decibel rating is cancelled out. Plus a lot of specialized units have radio comms so they can all speak to each other via radio piped into their earpro instead of just yelling at the top of their lungs.
One of the main reasons they (US Military) adopted the NGSW with a suppressor as part of the kit is because of Vet claims due to hearing loss. Suppressors are still loud with supersonic ammo but nowhere near as bad as cranking off 1,000 rounds of 5.56 in 24 hours with bare ears.
