for me it's the vanilla anomaly mechanics on ironman. chimeras will still fuck you, no matter what. you'll think you're prepared and then you find a pack of cats with a black chimera gang leader. cats will hold you while the chimera stretches your behind. also faction leaders give special hunt missions at night. mercs give a double black chimera in red forest, duty gives a truck cemetery quest, freedom got a bloodsucker village quest, trapper got some, the hunter at the garbage (forgot his name) also. even with exos and "empties" you'll still get 2-shot by chimeras (guess it depends on your difficulty settings). doesn't matter what equipment you have, you can never relax. death is always only a few unlucky circumstances away
